Non-citizen National Passport Card
Also known as a 5 star (or 4 star) Passport Card
By submitting a new DS-11 passport form you can correct your status.
Even if you have a valid passport you can still fill out a DS-11. You will check only the passport card box, send your valid passport in with a note on it "Still valid, please do not destroy."
You want the DS-11 form over the DS-82 renewal form because you need to update your citizenship answers which do not appear on the renewal form.

Do I need to go to an embassy?
It's often recommended but from my experience, no. It took me two different post offices and I did cross over to Nevada (out of a hunch, mainly). I did call beforehand to feel out the post office staff and vibe on my second attempt. Which was successful.
Passport Codes
One part of the application that can be used to your benefit is the box in the upper right hand corner. They leave out the instructions and make it appear to be official use only but it's not. Notice it is in the same tan section that is for you to fill out. The endorsement codes can be found on the U.S. Dept of State - Foreign Affairs Manual website. Code 09 is for "THE BEARER IS A UNITED STATES NATIONAL AND NOT A UNITED STATES CITIZEN."
You do not need to write anything in the white rounded edges box. You do not need to check any of the boxes. You put your two digit code on the line under the small boxes where it says "End #" which is appears to be short for endorsement. The instructions for using the endorsement codes is listed on the Foreign Affairs Manual.
